P.R.I.D.E. Recognition Form
All Employees and Individuals will be eligible for one monthly Pride Prize. Employees or Individuals earn a Pride Prize by being nominated or recognized by a fellow employee or fellow individual. Each person can be nominated by displaying, demonstrating or representing Perseverance, Respect, Integrity, Diversity and or Excellence.
When nominating someone, please give a brief description of the DSPs actions that exemplify PRIDE.
